Heart & Diabetes · Dr Manoj Gerela
Blood pressure treatment in Mulund and Bhandup
High blood pressure rarely announces itself, so it is usually found on a routine reading. Dr Manoj Gerela, cardiologist and diabetologist, sees hypertension at all three Good Health locations. 24-hour ambulatory monitoring is done from the Mulund East clinic, and every patient leaves knowing how to take a home reading the doctor can trust.
Is this for you
Come in if any of these apply
- A high reading at a pharmacy, a health camp or a pre-employment check, and nobody has looked into it since.
- Blood pressure tablets you have taken for years without a review of whether they are still the right ones.
- Readings that are fine at home and high at the clinic, or the other way round.
- Blood pressure together with diabetes, high cholesterol or kidney trouble, where the targets are stricter.
- Headaches, giddiness or nosebleeds that someone told you to get your pressure checked for. A proper reading is the first step.
- A parent or sibling with high blood pressure or an early stroke or heart attack.
What happens at the visit
Measure it properly, find out why, then treat
A reading done right
You sit and rest first. The reading is taken seated, back supported, arm at heart level, and repeated. Dr Manoj then takes a history of your health, medicines, family and habits, and examines you.
Tests, only where they change the answer
An ECG, and where indicated a 2D echo, to see whether the pressure has affected the heart. Blood and urine tests for the kidneys, sugar and cholesterol at Mulund East. Where clinic readings are not conclusive, a 24-hour ambulatory monitor is fitted.
The plan
Salt, weight, activity, sleep, alcohol and smoking are discussed honestly, because they move the number. Medicines are started or adjusted only if needed, with the reason explained. You leave with a target, a home-reading routine and a date to come back.
Home readings
How to take a reading the doctor can use
A home monitor is useful only if the readings are taken the same way each time. This is what Dr Manoj asks his patients to do.
- Use an upper-arm cuff rather than a wrist one, and bring it in once so it can be checked against ours.
- No tea, coffee, smoking or exercise for half an hour before. Empty your bladder.
- Sit quietly for five minutes. Back supported, feet flat, arm resting on a table at heart level, cuff on bare skin.
- Two readings a minute apart, morning and evening, for a week before your visit. Do not talk during the reading.
- Write every reading down and bring the whole list. One high number on a bad day tells us less than a week’s pattern.
Follow-up
Blood pressure is reviewed, not just prescribed
When a medicine is started or changed, Dr Manoj usually sees you again within a few weeks to check it is working and not causing side effects. Once your pressure is steady, visits space out and your home readings do most of the monitoring.
High blood pressure, diabetes and cholesterol travel together and damage the same organs, so your review looks at all of them. If you already see Dr Manoj for diabetes, it is the same visit. An annual kidney check and ECG is typical long-term.
Who does it
In clinical practice since 2001. Post-graduation in cardiology and diabetology at the Asian Heart Institute, Mumbai, and a fellowship in 2D echocardiography from the American Society of Echocardiography. He treats high blood pressure as part of the heart, not separately from it.

Questions about blood pressure
My pressure is only high at the clinic. Do I really have hypertension?
Possibly not, and this is what 24-hour ambulatory monitoring is for. The cuff records through a normal day and night away from the clinic, so Dr Manoj can see whether the pressure is genuinely raised. It is fitted at Mulund East and returned the next day.
Can I stop my tablets once the readings are normal?
Normal readings on treatment usually mean the treatment is working, not that it is finished. Do not stop or change a dose on your own. Bring your home readings to the review and Dr Manoj will decide with you whether anything can be reduced.
Should I take my tablet on the morning of the visit?
Yes, take your medicines as usual unless told otherwise, and bring the boxes. If a fasting blood test is planned, skip breakfast but not the tablets. Call the desk if unsure.
